Substance Use Disorder Counselor

Compensation is not specified. The Substance Use Disorder Counselor role is onsite in Hagerstown, IN, with a Tuesday–Saturday schedule from 8:00 AM to 4:30 PM. The counselor supports Recovery Works Cambridge City, part of Pinnacle Treatment Centers’ addiction treatment services and mission to remove barriers to recovery. The role provides evidence-based counseling and supports continuity of care from admission through discharge and follow-up. Through assessments, individualized treatment planning, therapy, documentation, and coordination with the multidisciplinary team, the counselor contributes to comprehensive patient care.

What you'll do

• Conduct substance use assessments using DSM criteria and ASAM guidelines
• develop and implement individualized treatment plans
• provide individual, group, and family counseling
• reassess patients and revise plans as clinically indicated
• develop discharge and continuity-of-care plans
• document services in the EHR
• coordinate with referral sources and community providers
• facilitate team-based crisis intervention
• maintain a caseload and comply with applicable standards.
