What you'll do

• Conduct in-home assessments for Long-Term Personal Care Services (LT-PCS) applicants.
• Evaluate client needs to support eligibility determinations and care planning.
• Provide Choice Counseling for Home and Community-Based Services (HCBS).
• Educate applicants and families on Medicaid programs and community resources.
• Develop and maintain person-centered Plans of Care.
• Coordinate services with healthcare providers and community organizations.
• Complete monitoring visits, client surveys, and case documentation.
• Ensure adherence to state and program guidelines.
