What you'll do

• Provide skill-development interventions for daily living, social functioning, and community integration
• document progress and services
• coordinate with internal and external providers
• support healthcare scheduling, follow-ups, wellness planning, and health literacy
• conduct regular home visits
• connect clients with community resources, educational opportunities, and natural supports.
