What you'll do

• Connect patients and community members to healthcare and support services
• assist with applications and enrollment for Medicaid, ACA Marketplace plans, Sliding Fee Scale, Medication Assistance Programs, and SHIIP
• conduct social health assessments using motivational interviewing
• provide health education, outreach, and presentations
• build relationships with staff and community stakeholders
• participate in health fairs and outreach activities
• document services and referrals
• support quality improvement and program compliance
• and collaborate with staff on workflow.

Why this role
