Public Health Program Director — Rural Health Transformation

The role connects program, technical, and community partners while monitoring implementation, contracts, and performance data.

What you'll do

• Oversee the enhanced care coordination and Welcome Home Kit project
• manage grant-funded contracts, scopes, budgets, expenditures, and deliverables
• provide technical assistance to grantees
• monitor process and outcome data, reports, and evaluation activities
• collaborate on program evaluation and software procurement and development
• support sustainability planning
• conduct site visits and stakeholder meetings
• coordinate with internal programs and external partners.

Why this role

Typical hours are Monday–Friday, 8:00 AM–4:30 PM, 37.5 hours per week. Travel within assigned regions and statewide is required as needed; Nashville office travel is not reimbursed.
