The Program Manager, Community-Based Services works onsite in Chicago, Illinois. Salary is $100,000 per year. Friend Health–HRDI’s community-based behavioral health programs serve people with complex behavioral health needs through ACT, CST, and FDDP services. The role provides clinical and operational leadership to strengthen program quality, access, engagement, compliance, and continuity of care. It oversees program leaders and performance, supports high-risk clients, and coordinates with internal teams and community partners to sustain effective services.

What you'll do

• Provide clinical, administrative, and operational oversight of ACT, CST, and FDDP programs
• supervise program supervisors and monitor quality, productivity, documentation, census, and outcomes
• review IM+CANS assessments, treatment plans, progress notes, and other records
• oversee high-risk client consultation and follow-up after crises or hospitalization
• manage referrals, enrollment, engagement, and capacity
• develop community referral partnerships
• collaborate with Intake, Quality, Billing, Finance, and leadership
• support staff recruitment, training, retention, and field safety.
