Job overview

The Case Manager – Youth Behavioral Health role is onsite at the Bartlett Primary Assessment Center in Bartlett, TN. Compensation is $44,000–$46,500 per year based on experience. Working under the clinical supervision of a licensed mental health professional, the case manager coordinates treatment activities, communication, and planning with youth, families, and partner agencies. The role supports preparation for reintegration and appropriate subsequent placement.

What you'll do

  • Provide case management and communicate with youth, families, and Juvenile Probation Officers
  • complete or coordinate mental health and substance abuse assessments
  • develop and update treatment, aftercare, performance, and discharge documents
  • support post-release outcome tracking
  • liaise with the program, DJJ, families, and external agencies
  • prepare families for visitation, home leave, aftercare, and meetings
  • connect families to community resources
  • participate in audits and reviews.
