This clinical position exists to identify and assess the nutritional needs of assigned patients and residents. The role involves initiating interventions to maintain nutritional status and enhancing the quality of life for individuals consistent with their personal health goals.

What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, weight changes, and significant health changes.
• Create and update care plans for nursing staff, complete MDS documentation, perform nutrition-focused physical exams, conduct family care conferences, and participate in interdisciplinary clinical meetings and QAPI quality assurance audits.
