The Home Care Physical Therapist, Per Diem (Ocean County) role is onsite, providing in-home care in Ocean County, NJ, including Toms River. Pay is $72–$95 per visit. Metro Physical & Aquatic Therapy provides physical, occupational, and speech therapy, massage, and acupuncture services. The therapist evaluates patients, develops individualized treatment plans, and provides one-to-one care in patients' homes. The role contributes to patient care by documenting visits, adjusting treatment based on outcomes, and educating caregivers; the schedule is flexible, and patients may be seen any seven days of the week.

What you'll do

• Accept and treat patients within the designated geographic area
• provide one-to-one treatment
• evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• deliver care consistent with professional standards, agency policies, and prescribed plans
• educate family members, home health aides, and other caregivers on therapy techniques
• perform and document initial examinations
• provide timely and accurate visit documentation
• evaluate treatment effects and adjust treatment
• treat geriatric patients
• travel to patients' homes within the assigned territory.
