What you'll do

• Oversee clinicians providing care in patients’ homes
• respond to incoming calls and follow up on patient-care needs
• coordinate training and educational programs
• assess staff learning needs and set development goals
• serve as a resource for clinicians and assist with escalations
• coordinate at least bi-weekly case conferences
• review cases, services, records, diagnoses, medications, procedures, and clinical course
• guide clinicians on therapeutic goals, priorities, and plans of care.
