What you'll do

• Manage the population health module and patient registries
• develop care plans with providers, care teams, patients, and families
• monitor progress
• meet with patients and families by phone or in person, with occasional home visits possible
• use motivational interviewing and teach-back
• coordinate care for high-risk patients
• analyze patient data
• work care-gap reports and chart reviews
• educate patients and staff
• support continuous quality improvement.
