Designated Coordinator, 245D

The Designated Coordinator, 245D role is onsite in Saint Paul, MN, with frequent visits to residential and community settings. Pay is $25 per hour. Assured Wellness provides licensed services to people receiving assistance through Traditional Waivers and aims to deliver quality care with respect, compassion, and knowledge. This role coordinates and evaluates person-centered services, ensuring support plans are implemented, staff are trained and competent, and services produce measurable outcomes. It also contributes to quality improvement through documentation, reviews, and evaluation of support and risk-management plans.

What you'll do

• Oversee implementation of individual support plans and addendums
• facilitate services toward measurable outcomes
• instruct and support Direct Support Professionals
• observe service delivery and verify staff competency
• train or delegate training
• maintain records
• analyze progress
• participate in reviews, audits, and quality improvement
• develop and evaluate support plans, risk-management and behavior-support strategies, and outcome-tracking systems.
