PCHH Community Health Worker (CHW)

What you'll do

• Act as a liaison among medical providers, nurse case managers, clinical and referral staff, insurance companies, agencies, health care coaches, and community health nurses
• assist with patient enrollment and discharge
• schedule appointments and follow up after hospital or emergency-room visits
• make appointment reminders and contact patients who miss appointments when necessary
• support care coordination and self-management goals
• assist with referral tracking, patient contact reports, and chronic disease education group sessions
• attend required meetings and help prepare monthly case-management reports.
