Social Service Director

Compensation is $55–$60 per year. The Social Service Director role is onsite in Downers Grove, IL. The role supports the Social Services Department at Pearl of Downers Grove, part of Pearl Healthcare’s post-acute rehabilitation services. The director leads the department’s operations and coordinates assessment, care planning, discharge planning, and referrals so patients’ social, emotional, and psychological needs are addressed in collaboration with families and the care team.

What you'll do

• Supervise, coordinate, and perform Social Services Department activities
• assess, plan, implement, and evaluate patients’ social, emotional, and psychological needs
• complete state-required documentation
• coordinate discharge planning and referrals
• maintain relationships with families and residents
• direct care plan meetings
• communicate with nursing, physicians, admissions, and other team members.
