What you'll do

• Deliver nursing services in patients’ homes and provide case management
• assess patients and monitor progress and clinical outcomes
• develop and monitor plans of care with patients or representatives
• document care, evaluations, orders, and coordination
• communicate with families, physicians, and other disciplines
• participate in interdisciplinary conferences and patient education.
