What you'll do

• Guide patients through care pathways and identify delays or barriers
• serve as a point of contact for families and communicate service wait times
• create scheduling plans aligned with client needs and provider capacity
• coordinate with clinical teams and site leadership
• support contract and value-based care scheduling
• address client inquiries
• use scheduling tools including Axon, Smart Scheduling, and Salesforce
• and monitor scheduling performance indicators.
