What you'll do

• Assess patients’ health and needs in their homes
• develop and implement individualized care plans
• educate patients and families about conditions, medications, and self-care
• assist with discharge planning
• monitor progress and adjust plans
• coordinate with physicians, social workers, and therapists
• document interactions in accordance with ICD-10 standards
• provide health coaching
• provide specialized services such as esthetic laser treatment when applicable
• support family-planning initiatives and education.
