What you'll do

• Guide treatment plans with the interdisciplinary team
• coordinate tentative discharge and contingency plans
• participate in discharge planning and execution
• facilitate weekly team conferences and treatment plan modifications
• complete case management documentation
• review reports and payor requirements
• meet with patients and families within required timeframes
• arrange referrals and resources
• participate in utilization review and the case management on-call schedule as needed.

Why this role

Must be qualified to independently complete assessments within the scope of the relevant discipline and hold an active license when required. Nursing candidates need at least an associate degree in nursing; other eligible licensed or certified professionals need at least a bachelor’s degree. Two years of rehabilitation experience preferred.
