What you'll do

• Implement products and ACO initiatives with market leadership
• support wellness visits, transitions of care, and active management
• help practices meet mutually developed transformation plans
• train office staff and providers on processes and technologies
• assess training needs
• build relationships with providers and healthcare executives
• listen to practice concerns and relay improvement opportunities
• travel up to 25% across the Maryland/Delaware market for local practice visits.
