Job overview
The MDS Assessor role is onsite in Ditmas Park, Brooklyn, NY, with a Monday–Friday, 9:00 AM–5:00 PM schedule. Salary is $120,000–$130,000 per year. The assessor joins the clinical team at Ditmas Park Nursing & Rehab. The role completes resident assessments and supports individualized care planning, regulatory compliance, and accurate documentation. It coordinates with interdisciplinary departments so assessments reflect residents’ conditions and support appropriate care and reimbursement.
What you'll do
- Complete MDS assessments accurately and on time
- review resident documentation and clinical records
- coordinate with nursing, therapy, social services, dietary, and other departments
- participate in interdisciplinary care-plan meetings
- ensure assessments reflect resident condition and level of care
- monitor documentation for accuracy and compliance
- support reimbursement under PDPM
- maintain compliance with CMS and New York State requirements.
