What you'll do

• Provide evidence-based assessment, intervention, consultation, and referral
• collaborate with the interdisciplinary team and develop individualized treatment goals
• create and facilitate behavioral health, substance use, and chronic disease care plans
• support primary care providers with screening, identification, diagnosis, and management
• participate in quality improvement
• provide consultation and training support
• document encounters in the electronic medical record
• maintain licensure and clinical competency.
