Pediatric Home Service helps medically complex children receive care at home, school, and in their communities. Nurses provide individualized skilled care to children and young adults with complex medical needs, following care plans and communicating changes to the care team. One-to-one assignments support focused care and continuity for patients and their families.

What you'll do

• Provide skilled nursing care in home and school settings
• assess and monitor patient condition and communicate changes to the care team
• administer medications, treatments, and prescribed therapies
• provide tracheostomy, ventilator, airway, and G-tube care as needed
• document care and educate families
• follow individualized care plans and maintain a safe environment.
