Description

Under the direction of the Program Manager, the Time Limited Subsidy Case Manager formerly known as Rapid Rehousing Case Manager will provide services to homeless transition aged youth (18-24 years old). The Rapid Rehousing Case Manager will be responsible for assessing needs and connecting youth to housing that is appropriate for their needs. This may include (but not be limited to) crisis/bridge
