Child Case Manager

Job overview

The Child Case Manager role is onsite in Iola, KS, covering Allen and Neosho Counties; pay is $18.92 per hour. The position supports Children’s Services and Case Management at Southeast Kansas Mental Health Center. The case manager provides community-based, goal-directed support and coordinates services around each patient’s treatment plan. The work helps children and families access medical, educational, and other supports, strengthen independent functioning, and coordinate care across family and community systems.

What you'll do

• Provide face-to-face community-based interventions
• help patients identify strategies and treatment options
• support emotional and behavioral management and crisis planning
• coordinate treatment planning with patients, therapists, families, and supports
• connect patients with medical, educational, housing, employment, and other services
• facilitate wraparound teams
• document services in the electronic health record and submit patient status reports.
