What you'll do

• Conduct patient assessments and develop individualized plans of care
• manage pain and symptoms
• coordinate with physicians, social workers, and chaplains
• educate and support patients and families
• make skilled nursing visits in homes, assisted living facilities, or other care settings
• document care, medication administration, and clinical notes
• respond to after-hours calls and provide on-call nursing support as needed.
