Substance Use Case Manager

The Substance Use Case Manager role is based in Burlington, VT, with services delivered in DCF offices, community spaces, and caregivers’ homes. The starting rate is $25 per hour. Lund’s Regional Partnership Program works with DCF Family Services to support families and improve outcomes for children affected by parental substance misuse. The case manager screens for substance use concerns, connects parents with assessment and treatment services, and helps address barriers to engagement. The role also consults with and trains DCF staff and coordinates between family services and treatment providers to support access to care.

What you'll do

• Conduct substance use screenings
• coordinate referrals to assessment and treatment providers
• support treatment engagement by addressing transportation, childcare, and stigma barriers
• consult with and train DCF staff on substance use and treatment services
• liaise between treatment providers and DCF Family Services
• provide services in offices, community settings, and caregivers’ homes.
