Registered Nurse (RN) Case Manager – Truman Health Services

The Registered Nurse (RN) Case Manager is an onsite, full-time, 40-hour-per-week day-shift role in Albuquerque, NM. Compensation is $70,849–$90,302 per year, based on relevant experience. The role supports Truman Health Services, whose interdisciplinary team provides HIV and gender-affirming primary care, along with Hep-C and PrEP consult services. This position coordinates comprehensive clinical and community-based care to improve quality of life for patients and families. The nurse assesses and tracks patient needs, develops care plans with the care team, supports continuity across settings, and provides nursing care and follow-up so patients can remain connected to treatment.

What you'll do

• Coordinate patient care with the multidisciplinary team
• identify patients needing case management through data, chart review, screenings, and referrals
• maintain tracking systems and records
• develop and update collaborative care plans
• coordinate continuity of care and hospital discharge follow-up
• assist with assessments and triage
• conduct nurse intakes and visits
• provide patient education, wound care, medication administration, and other RN-scope care
• conduct hospital visits and participate in home visits as needed
• respond to emergencies.
