What you'll do

• Provide information and referral, advocacy, peer support when applicable, skills training, and transitional services
• follow up with consumers and service providers
• manage schedule and caseload as appropriate
• participate in agency meetings and professional development
• travel within the 13-county service area
• evaluate homes, vehicles, employment sites, and daily living needs for assistive technology and home modifications
• prepare detailed reports.
