The Hospice and Palliative Care Physician role is onsite in Titusville, FL, serving patients and families throughout Brevard County. Compensation is not specified. St. Francis Hospice’s interdisciplinary care team provides care intended to support dignity and comfort for people facing life-limiting illness. The physician directs the medical component of hospice care, evaluates admissions and ongoing eligibility, and works with primary physicians and the interdisciplinary team to coordinate comprehensive care. Clinical leadership, medical guidance, and required documentation support safe, compassionate care for patients and families.

What you'll do

• Evaluate hospice admissions and eligibility, including relevant comorbidities
• round on and supervise inpatient hospice patients, including weekend coverage as needed
• lead and participate in interdisciplinary group meetings and oversee care plans
• prescribe and oversee pharmacotherapeutics in collaboration with care partners
• conduct face-to-face eligibility visits
• make community home visits for medical management and follow-up
• complete documentation, billing, and death certificates within Florida’s 72-hour statutory requirement
• provide after-hours, weekend, and holiday on-call coverage
• participate in QAPI and support regulatory compliance.
