Care Concierge — Fort Lauderdale to Boca Raton

The Care Concierge role is onsite and requires the employee to be located in and able to report to an office between Fort Lauderdale and Boca Raton, FL. Compensation is not specified. The role supports Carewell Family Services’ care navigation program for members and families. The concierge moves among enrollment, welcome visits, and care navigation according to member needs and queue volume. By building trust, coordinating with providers and community resources, and following care gaps through to resolution, the role supports member access to services and a consistent care experience.

What you'll do

• Conduct outreach to prospective members, explain the program, obtain and document required verbal consents, complete intake questionnaires, and schedule initiating provider appointments
• conduct welcome visits and provide condition-specific education and self-management coaching
• coordinate with providers, pharmacies, home health agencies, and community resources
• support referrals, prior authorizations, medication access, and social needs screenings
• document interactions and escalate concerns.
