What you'll do

• Provide direct care based on individual needs and service plans
• support personal care, daily living activities, meal preparation, medication support, household routines, community outings, recreation, and skill development
• monitor health and report changes
• complete documentation
• collaborate with team members. In ICF settings, the role may include total-care assistance and Hoyer-lift transfers.
