Care Manager

What you'll do

• Identify community members needing medical, behavioral health, and social services
• contact assigned participants within three days
• maintain an active caseload of 45 or more
• prepare participant care plans
• obtain services according to participant needs
• maintain regular contact, including home visits as needed
• keep accurate service records
• attend meetings and training
• represent the program at assigned community activities
• work outside regularly scheduled hours as needed.

Why this role

Requires bilingual Spanish and English, a GED or associate degree in human services or a related field, at least two years of experience working with Latinos connecting them with community services, a valid New York State driver’s license, and personal transportation. Equivalent training and experience may be accepted.
