What you'll do

• Participate in referral matching and advocate for appropriate placements
• develop and facilitate treatment plans and assessments
• refer children to support services
• ensure required legal documentation and safety plans
• support foster parents and children in implementing plans
• monitor required services, contacts, records, and medication logs
• coordinate with placement agencies, schools, families, and community providers
• complete required home visits and discharge planning
• transport clients as needed and travel between locations
• participate in a weekly rotating on-call schedule and document related activities.
