What you'll do

• Coordinate treatment and continuity of patient care
• maintain a caseload of approximately 12 patients
• complete assessments, treatment plans, progress notes, and discharge documentation
• provide individual, family, or group counseling and other services specified in treatment plans
• coordinate referrals and multidisciplinary care
• assist with problem solving and audit readiness. Coverage for the Clinical Director may be provided during absence, and interim director duties may be performed when the Director is off-site.
