What you'll do

• Gather and analyze information and complete admission forms
• evaluate patients for hospice appropriateness, including medical record review as requested
• obtain agreement for admission from eligible patients and families
• analyze collected data to facilitate admission orders
• build referral-source relationships
• maintain clinical competence
• develop internal relationships with admissions and patient care resources
• educate patients and families about hospice and palliative care.

Why this role
