The nurse assesses and manages patient and family needs, provides interventions and education to support comfort and quality of life, and coordinates care with the interdisciplinary team. The role also supervises assigned field staff and communicates patient status and care needs to colleagues and management.

What you'll do

• Case-manage assigned patients and families
• assess physical, psychological, social, and spiritual needs and initiate interventions
• supervise LPNs, CNAs, and other assigned staff
• assist with staff orientation
• address patient and family concerns
• educate the community about hospice
• create and update nursing care plans
• complete documentation
• participate in interdisciplinary group activities
• collaborate with the team physician and Clinical Care Manager.
