The Afterhours Registered Nurse (RN) Care Manager is an onsite role based in Waverly, AL. This position provides high-quality, compassionate hospice care outside standard business hours. This role exists to ensure seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. The RN Care Manager assesses patient needs, coordinates care, and serves as an active member of the Interdisciplinary Group.
