What you'll do

• Provide one-on-one home health care during the shift
• assist with daily living activities
• observe the patient and record significant conditions and reactions
• follow the patient care plan and update it as needed
• administer prescribed medications and treatments and document effectiveness
• respond to changes in condition and report them to the appropriate physician or supervisor.
