The nurse develops and carries out home-based plans of care to promote wellness and help prevent rehospitalization. Through patient and caregiver education, clinical assessment, documentation, and coordination with an interdisciplinary team and physicians, the role supports safe, effective care throughout each patient’s care episode.

What you'll do

• Assess patients and families at initial visits
• develop plans of care, including rehabilitation
• work one-on-one with patients and families toward improvement or stability
• case-manage care episodes with the interdisciplinary clinical team
• prepare clinical notes, orders, and charting within required timeframes
• collaborate with physicians and agency staff
• oversee assistance and teaching related to wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication education.

Why this role

Field visits to patients’ homes are required. Schedule is Tuesday–Saturday or Sunday–Thursday; must be willing to work Saturday or Sunday. Requires current unrestricted California RN license, BLS/CPR certification, and valid California driver’s license. Occasional coverage outside the primary territory may be requested.
