The Clinical Liaison — Community Relations & Business Development role is onsite in Asheville, NC, with regular in-person visits to healthcare and senior-care partners throughout the surrounding area. Pay is $60,000–$85,000 per year. At Home Harmony aims to expand access to in-home care for older adults, people living with dementia, and families needing support. The representative builds referral relationships, develops a market growth plan, and generates referrals for in-home primary care, dementia care, transitional care, and GUIDE Model services. Coordination with clinical and intake teams helps create a seamless experience for referral partners and new patients.

What you'll do

• Build referral relationships with senior living communities, hospitals, skilled nursing facilities, home health agencies, hospice providers, rehabilitation facilities, and other healthcare partners
• develop and execute an Asheville market growth plan
• generate patient referrals
• visit partners in person
• represent the organization at networking and community events
• track referral activity, conversions, and market trends
• coordinate with clinical and intake teams.
