What you'll do

• Analyze panel reports to identify trends, gaps, and process improvements
• conduct pre-visit panel assessments and support care-team communication
• manage daily patient schedules and triage work-ins
• monitor provider panel balance and patient status
• coordinate quality improvement strategies
• maintain knowledge of provider roles and EMR documentation standards
• educate new patients about the Medical Home concept, Patient Portal, ancillary services, and team-based care
• liaise among patients, clinical teams, operations, and support departments.

Why this role

Requires a current North Carolina RN/LPN license or a BS in a health-related field, plus BLS Provider credentialing. One year of public health or ambulatory care experience is preferred.
