What you'll do

• Conduct participant intake and orientation, coordinate admissions, and maintain required releases and confidentiality documents.
• Coordinate behavioral health, medical, housing, employment, educational, and vocational services and referrals.
• Develop and update Individualized Care Plans, coordinate assessments, and maintain timely, accurate records in compliance with HIPAA, 42 CFR Part 2, and CSOSA requirements.
• Monitor attendance and engagement, support transitions and discharge planning, and communicate with clinical staff, Community Supervision Officers, and service providers.
