What you'll do

• Complete initial patient and family assessments and histories
• regularly reevaluate nursing needs
• initiate and revise plans of care as patient needs change
• develop care plans with patient and family involvement
• provide preventive and rehabilitative nursing procedures
• administer prescribed medications and treatments
• counsel and educate patients and families
• identify and implement discharge planning needs
• act as case manager when assigned.
