What you'll do

• Perform weekly wound assessments and measurements
• document wound stage, size, and progress
• provide treatments and dressing changes according to physician orders and best practices
• develop and update care plans
• identify pressure-injury risk using the Braden Scale and implement prevention plans
• coordinate with physicians and care team members
• educate nurses and CNAs
• track wound data and support quality reporting and MDS accuracy
• communicate changes to residents and families.
