What you'll do

• Assess spiritual and emotional needs
• develop and support individualized spiritual care plans
• provide spiritual counseling, emotional and grief support, and compassionate presence
• participate in interdisciplinary meetings and care planning
• collaborate with clinical staff and community clergy
• coordinate faith leader visits when appropriate
• maintain clinical documentation and participate in quality and compliance initiatives.
