Home Care Account Executive — Community Liaison

What you'll do

• Own a defined territory and grow referrals
• build and maintain relationships with case managers, social workers, community organizations, and Medicaid and VA referral sources
• attend and represent the agency at community events, health fairs, and outreach programs
• educate referral partners and families about home care services and support for clients and caregivers
• track outreach activity and referrals in the CRM
• work toward agreed-upon results.
