What you'll do

• Assess needs and strengths
• set goals and develop care plans
• provide bi-weekly counseling and psycho-educational groups
• provide monthly family support or counseling
• deliver solution-focused counseling related to substance-abuse risk factors
• provide crisis intervention and coordinate risk assessments
• provide case management, referrals, and advocacy
• partner with the academic team
• document progress, assessments, and transition plans
• participate in care reviews and enrollment decisions.
