Client Service Coordinator — Home Care

The Client Service Coordinator role is onsite in Fairfield, Connecticut, with pay of $25–$30 per hour. What you'll do

• Process referrals from intake, email, and phone
• match clients with appropriate caregivers and ensure cases are staffed
• monitor caregiver arrivals and arrange coverage for late, called-out, or no-show caregivers
• build relationships with clients and families
• respond to client and caregiver issues
• maintain client and caregiver information in the WellSky system
• communicate concerns to team members
• coordinate community referrals when appropriate
• participate in rotating paid evening and weekend on-call coverage.
