Blended Service Coordinator

The Blended Service Coordinator works onsite in the Greater Pittsburgh Area, Pennsylvania, with a listed rate of $22.50–$27.84 per hour. The coordinator assesses needs and organizes mental health services and supports so individuals can pursue recovery and safe community life. The role advocates for individuals, links them and their families to resources, maintains service records, and coordinates with a treatment and service team.

What you'll do

• Advocate for individuals to ensure responsive natural, community, geriatric, and specialized supports
• maintain timely service records, review charts for regulatory compliance, and prepare for audits
• help individuals and families identify, access, and coordinate resources
• provide on-site support for inpatient mental health commitments after alternatives to hospitalization have been considered
• provide culturally competent services
• attend training
• maintain contact based on individual needs, at least monthly, and provide a full range of coordination services to a designated caseload. Schedule is Monday–Friday, 9 a.m.–5 p.m., with some weekend on-call availability. Requires a relevant bachelor’s degree, at least one year of human-services experience, clearances, and a valid Pennsylvania driver’s license with required driving history.
