Wound Care Nurse (Registered Nurse or Licensed Practical Nurse)

The Wound Care Nurse role is onsite in Independence, MO. Compensation is $35 per hour for LPNs and $45 per hour for RNs. The nurse supports the skilled nursing team at Outfield Healthcare Partners, leading skin-integrity and wound-management programs for residents. The role exists to assess and treat wounds, monitor healing, prevent skin breakdown, and support regulatory compliance. Through accurate documentation, preventive interventions, staff education, and collaboration with nursing leadership and physicians, the nurse contributes to resident care quality and positive outcomes.

What you'll do

• Perform comprehensive skin and wound assessments
• obtain and carry out wound-care treatment orders
• monitor healing and recommend treatment changes
• document assessments, treatments, and outcomes
• track pressure ulcers and skin alterations with weekly measurements and reports
• ensure Braden risk assessments are completed accurately
• implement and monitor preventive interventions
• educate nursing staff on wound-care protocols
• coordinate supplies
• assist the Director of Nursing and Medical Director with pressure-injury case reviews
• participate in QAPI initiatives
• stay current on applicable wound-care standards.
